In- HIB INSURANCE BENEFIT
Provider: Prudential Guarantee and Assurance, Inc.

This coverage represents as payment for the loss of income of a member should there be confinement occurred due to sickness or accident with a maximum coverage of 60 days per confinement with an aggregate amount of 365 days in a year.

In-HIB shall not cover sickness due to pre-existing conditions unless the insured has been covered for the past (12) consecutive months. A sickness or condition is considered to be in any of the following cases:

• Such sickness or condition was in any way evident to the insured before the effectivity date of the Policy and/or Certificate of Insurance (COI).

• The natural history/pethogenesis of such sickness or condition can be clinically determined to have started prior to the effectivity date of coverage whether or not the Insured is aware of such sickness or condition.

DENTAL HEALTH PARTNERS ACCESS CARD
Provider: Health Partners Dental Access, Inc.

A member can avail unlimited dental services of:

• Oral examination/diagnosis as needed
• Gum problem consultation and treatment planning
• Simple tooth extraction when indicated (excluding impaction)
• Temporary fillings when indicated
• Re-cementation of loose jacket crowns
• Adjustment of dentures
• Oral hygiene instruction
• Orthodontic treatment consultation
• Dental health education and consultation
• Oral prophylaxis every six (6) months (twice a year)
• Two (2) surfaces of permanent fillings annually (any of the following that is needed
• Amalgam filling, Light Cure Composite or Glass lonomer)

A discount of 10% for other services not covered by the dental plan (e.g. dentures, x-rays, permanent fillings and dental surgery)
