- The Gold Health Circle Plus is an annual health and wellness membership.
- The Gold Health Circle Plus is not transferable.
- Always present your card to avail of the benefit.
- Damaged card can be replaced free of charge upon surrendering the damaged card to our office. Lost card can be replaced with a certain amount of payment for administrative cost.
- Hospital Income Benefit (HIB) can be assigned within 30 days only from the date of coverage. Dental Card is not transferable.
- Cancellation of coverage is on discretion of the management. A cancellation fee of Php 1,700.00 shall be collected before the issuance of refund letter advise to our payment gateway online merchant.
- GD Prime Marketing Services reserves the right to amend these guidelines without prior notice.
- Our billing descriptor is GD MKTG 5344798. This will appear on your credit card billing statement depending on your bank cut off date.
